These prices are effective as of January 1, 2005. Prices are subject to change without notice.

GENERAL INFORMATION: DISCLOSURES
The goods and services shown below are those we can provide to our customers. You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we would explain the reason in writing on the statement we provide describing the funeral goods and services you selected.
Other funeral service providers may provide certain funeral services off premises.
This list does not include the price for certain items that you may ask us to buy for you such as crematory services and newspaper notices. The prices for these items will be shown on your bill or the statement describing the funeral goods and services you selected.


I. PROFESSIONAL SERVICES OF FUNERAL DIRECTOR AND STAFF

Basic Services of Funeral Director and Staff          $ 1,570.00
Our fee for the services of funeral director and staff includes, but is not limited to, staff to respond to initial request for service, arrangement conference with family or responsible party, arrangement of funeral, preparation and filing of necessary authorizations and permits, recording vital statistics, preparation and placement of obituary notices, staff assistance prior to, during and following the funeral, including coordination with those providing other portions of the funeral, e.g. cemetery, crematory and others.
This fee for our basic services and overhead will be added to the total cost of the funeral arrangements you select. (This fee is already included in our charges for direct cremations, immediate burials, and forwarding or receiving remains.)

II. OTHER SERVICES
A. Funeral Home Facilities

1. Facilities and Staff for Visitation and Viewing $ 205.00
Our services include set-up of visitation area, placement of encased remains, display of floral arrangements, supervision of and attendance during the visitation.

2. Facilities and Staff for Funeral Ceremony $ 405.00
Our services include staff services for funeral service or additional staff necessary for service in other facility. In addition it includes our service in coordinating the funeral arrangements, supervision of funeral, and staff to attend ceremony.

3. Facilities and Staff for Memorial Service (without body) $ 405.00
Our services include coordinating the memorial service arrangements, supervision of the memorial service, and staff to attend the service.

4. Funeral Home Owned Equipment and Staff
for Graveside Service $ 220.00
Our services include accompaniment of remains to cemetery, supervision of graveside service and staff to attend service.

B. Embalming
Except in special cases, embalming is not required by law. Embalming may be necessary, however, if you select certain funeral arrangements, such as a funeral with viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as direct cremation or immediate burial.

D. Immediate Burial $ 1,655.00 to $ 8,725.00
Our charge for immediate burial, without any attendant rites or ceremonies, includes removal and shelter of remains, local transportation to the cemetery, necessary services of staff and authorizations.

E. Direct Cremation $ 1,720.00 to $ 5,725.00
Our charge for a direct cremation without any attendant rites or ceremonies includes local removal of remains, refrigeration of remains, crematorium charge, local transportation to cemetery, necessary services of staff and authorization.

State and local laws do not require a casket for direct cremation. If you want to arrange a direct cremation you can use an alternative container. Alternative containers encase the body and can be made of materials like fiberboard or composition materials (with or without an outside covering). The containers we provide are constructed of cardboard.

F. Transfer of Remains to Funeral Establishment

1. Within Local Area (1-19 miles) $ 180.00

2. Within Surrounding Area (20-45 miles) $205.00

3. If transportation is required beyond stated surrounding areas (45+ miles), we charge a flat rate of $205.00 for the first 45 miles plus $2.15 per each additional loaded mile.

G. Forwarding Remains to Another Funeral Establishment $ 1,165.00 to $ 8,235.00
Our charge includes local removal of remains, necessary services of staff, normal embalming, dressing of remains, necessary authorizations and transportation to Norfolk International Airport. This charge does not include visitation rites or ceremonies prior to forwarding of the remains.

Our charge includes temporary shelter of remains, transportation from airport, local transportation of remains to cemetery, and necessary services of staff. This charge does not include visitation rites, ceremonies, dressing of remains, or hairdresser.

J. REMEMBRANCE SERVICE $ 730.00
Our charge for a remembrance service includes the professional service fee for the funeral director to coordinate and supervise the remembrance service, as well as our facility fee.

In most areas of the country, state or local law does not require that you buy a container to surround the casket in the grave. However, many cemeteries require that you have such a container so that the grave will not sink in. Either a grave liner or a burial vault will satisfy these requirements.
