1. BASIC SERVICES OF FUNERAL DIRECTOR AND STAFF
Our charge includes consultation with the family or responsible party; staff and facilities to respond to initial request for service; coordinating service plans with the cemetery, crematory, and/or other parties involved in the final disposition of the deceased; securing official documents and necessary registrations and sheltering of the deceased. This fee for our basic services and overhead will be added to the total cost of the funeral arrangements you select. (This fee is already included in our charges for direct cremations, immediate burials, and forwarding or receiving remains.)

2. EMBALMING
Except in certain special cases, embalming is not required by law. Embalming may be necessary, however, if you select certain funeral arrangements, such as a funeral with viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as direct cremation or immediate burial.

3. OTHER PREPARATION OF THE BODY
Cosmetology, dressing and placement of the deceased in the casket
Sanitary handling of unembalmed remains for private viewing by family

4. ADDITIONAL SERVICE AND FACILITY FEES
a. Use of facilities and staff for viewing
b. Use of facilities and staff for private
family viewing one hour prior to service
c. Use of facilities and staff for memorial
service
d. Use of the facilities and staff for funeral service
e. Use of staff for memorial service in a church or other facility
f. Use of staff and equipment for a funeral in a church or other facility
g. Use of staff and equipment for graveside service only

5. TRANSFER OF REMAINS TO FUNERAL HOME

6. AUTOMOTIVE EQUIPMENT
a. Use of hearse for transportation to place of disposition
b. Use of five passenger sedan
c. Use of service car for staff and or bearers, clergy and flowers
d. Additional mileage charge; Transportation beyond local area is charged at $2 per mile, one way. Unless otherwise noted, all transportation charges contained in the General Price List apply to local area only within 25 mile radius of funeral home.

7. CASKETS:
A complete price list will be provided at the funeral home.

8. OUTER BURIAL CONTAINERS:
A complete price list will be provided at the funeral home.

9. URNS:
A complete price list will be provided at the funeral home.

10. CLOTHING AND OTHER MERCHANDISE DO NOT OFFER

11. FORWARDING OF REMAINS TO ANOTHER FUNERAL HOME
This charge includes: Basic services of staff, embalming, initial transport of the deceased from place of death to our funeral home and local transportation. (Airline or other shipping charges not included)

12. RECEIVING OF REMAINS FROM ANOTHER FUNERAL HOME
This charge includes: Basic services of staff, care of the deceased and local transportation of the deceased to cemetery or crematory.

13. IMMEDIATE BURIAL
Our charge for an immediate burial (Without Ceremony) includes: Basic services of staff, initial transfer of the deceased from place of death to funeral home and local transportation of the deceased to the cemetery.

14. DIRECT CREMATION
Our charge for direct cremation (Without Ceremony) includes: Initial transfer of the deceased from the place of death, basic services of staff and local transportation to the crematory.

If you want to arrange for direct cremation, you can use an alternative container. Alternative containers encase the body and can be made of materials like fiberboard or composition materials (with or without an outside covering). The containers we provide are constructed of corrugated cardboard.

These prices do not include the outside charges such as the crematory fee and the fee for the Medical Examiner Permit. These and any other items you may choose will be added to the above services.

15. BODY DONATION TO A MEDICAL SCHOOL OR FACILITY
Our charge for assisting in University of Vermont College of Medicine Anatomical Gift Program (Without Ceremony) includes: Basic services of staff, initial transfer of deceased from place of death and transportation to UVM College of Medicine.

16. OUR BASIC SERVICE CHARGE INCLUDES A $ 6.00 FEE THAT WILL BE SENT TO THE STATE OF VERMONT AND SET ASIDE FOR THE PROTECTION OF HOLDERS OF CONTRACTS FOR PREPAID FUNERAL ARRANGEMENTS.

17. Funds may be available for final disposition through the Vermont Department of Social Welfare, General Assistance Program, for those who qualify. You may contact them by calling 802 863-7365.

18. The funeral industry is governed by laws defining professional conduct. Consumers who have inquiries or wish to obtain a form to register a complaint may do so by calling 802-828-2367 or by writing the Secretary of State at 109 State St., Montpelier, VT 05609-1106.

19. ARRANGEMENTS FOR BURIAL IN THE SPRING
When burial cannot take place in Winter months this charge includes: Use of hearse from place of storage to cemetery and professional services and staff for committal service.

20. Additional charge per hour for services not included in any of the above charges
