THESE PRICES ARE EFFECTIVE AS OF JUNE 1, 2009
AND ARE SUBJECT TO CHANGE WITHOUT NOTICE. THE CURRENT PRICES SHOWN REFLECT ALL PREVIOUS DISCOUNT CERTIFICATES, WHICH ARE NOW NULL AND VOID BY THE U.S. FEDERAL TRADE COMMISSION FUNERAL RULE. PREVIOUS DISCOUNT CERTIFICATES ARE THEREBY INVALID.

BECAUSE OUR FAMILY IS HERE TO SERVE YOUR FAMILY REGARDLESS OF THE TYPE OF SERVICE OR MERCHANDISE YOU SELECT, WE PLEDGE TO ASSIST YOU TO THE FULLEST EXTENT POSSIBLE WHILE TAKING INTO ACCOUNT YOUR INDIVIDUAL NEEDS.

(You will note that we offer Personalized Service Program Packages. The funeral director handling your arrangements will further explain our Personalized Service Program Packages.)

The goods and services shown below are those we can provide to our customers. You may choose only the items you desire.
However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected.

PROFESSIONAL SERVICES

A. Basic Services of Funeral Director and Staff------- $1250.00
Our charge includes, but is not limited to, consultations with the family or responsible party; staff and facilities to respond to initial request for service; coordinating service plans with cemetery, crematory, and/or other parties involved in the disposition of the deceased; securing official documents and necessary registration and sheltering of the deceased up to three days. Also included in this charge are general operating expenses (overhead) relative to our facilities such as insurance, maintenance, utilities, taxes, salaries and benefits. Other costs included are regulatory compliance, legal and audit fees, secretarial, administrative, equipment inventory and parking expense. This fee for our basic services and overhead will be added to the total cost of the funeral arrangements you select. (This fee is already included in our charge for direct cremation, direct burials, and forwarding or receiving remains.)

B. Embalming----------------------------------------------------------- $450.00
Except in certain circumstances, embalming is not required by law. Embalming may be necessary, however, if you select certain funeral arrangements, such as a funeral with viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as direct cremation or direct burial.

FACILITIES, STAFF, AND EQUIPMENT
This fee includes, but is not limited to, staff arrangement, planning and direction of the visitation and/or ceremony or selected service, use of facilities, use of equipment, and transfer to and from another local facility.

TRANSPORTATION
Unless otherwise noted, transportation charges on the General Price List apply to local area only (within a 30 mile radius of our facility). Our point to point charge to destinations is $1.50 per mile (one way). Expenses equal to those incurred for food and overnight lodging as required, will be added to this charge, on trips exceeding eight hours duration.

FORWARDING OF REMAINS TO ANOTHER FUNERAL HOME
Guideline Minimum Transfer----------------------------------------$1,550.00
Includes Basic Services(A), Embalming(B), Other preparation(C), as required, and Transfer to Mortuary(Q). These charges do NOT include minimum shipping container/air tray or any of the transportation costs. This selection does NOT include any viewing or ceremony.

Standard Transfer w/Local Viewing------------------------------$2,050.00
Includes Basic Services(A), Embalming(B), Other preparation(C), Limited viewing(K) and Transfer to Mortuary(Q). These charges do NOT include casket or shipping container/air tray or any of the transportation costs.

RECEIVING OF REMAINS FROM ANOTHER FUNERAL HOME
Guideline Minimum Delivery--------------------- --------------------$900.00
Includes Basic Services(A) and Final Transfer(T). These charges do NOT include cemetery merchandise or expenses or any transportation costs outside of a 30 mile radius. This minimum delivery option will be completed in accorrdance with our schedule without any viewing or ceremony.

Traditional Ceremony with Visitation at the Mortuary-----------$1850.00
Includes Basic Services(A), Visitation(J) and Funeral Ceremony(M), Use of Funeral Coach(R) Service Vehicle(U), and Transfer to Mortuary(Q). These charges do NOT include cemetery merchandise or expenses.

IMMEDIATE CREMATION PACKAGES
Direct Cremation-------------------------------------$1,300.00
Includes Basic Services(A), Alternate Care(D), as required, Transfer to Mortuary(Q), Service Vehicle(U) and crematory charge. These charges do NOT include a cremation urn, or do NOT provide for visitation or any ceremony. If you want to arrange a direct cremation, you can use an alternative container. Alternative containers encase the body and can be made of materials like fiberboard or composition materials (with or without an outside covering). The containers we provide are rigid cardboard boxes.

SPECIAL CONSIDERATION FOR YOUTH SERVICES
The following schedule is based on the same service fee quoted as Adult General Price List charges, with the following percentages to be
Infant - Stillborn to 1 month 20% adult
Infant 1 month to 1 year 30% adult
Infant 1 year to 3 years 40% adult
Child 3 years to 7 years 60% adult
Child 7 years to 12 years 80% adult
(Note: Percentage does NOT apply to merchandise, transport or other charges.)

MERCHANDISE

PERSONALIZED SERVICE PROGRAM PACKAGES
As an alternative to selecting itemized services, you have the option of choosing a packaged funeral service
arrangement. These packages must meet our payment policy procedures and are available only when merchandise
is purchased from our firm. These packages must be taken in their entirety, with allowable exceptions indicated, without
any other deletion or substitution. Additional items, such as motor equipment, can be included at G.P.L. cost. When
compared to itemized selections, these service options may offer savings to the purchaser as opposed to selecting the items individually. The following price quotations are provided to assist you during the consideration of funeral arrangements.

Includes Basic Services(A), Alternative Care(D), as required, Funeral service(O),Transfer to Mortuary(Q), and Service Vehicle(U). (NOTE: Cremation charge, if chosen to be the method of disposition, is not included. An alternative container may also be required)

IF PAYMENT IS TO BE MADE THROUGH A PROBATED ESTATE A CHARGE OF $250.00 APPLIES.
