| These prices are effective as of June 1, 2006 but are subject to change without notice. The goods and services shown below are those we can provide to our customers. You may choose only those items you desire. However, any funeral arrangement you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any item you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected. MINIMUM SERVICES OF THE FUNERAL DIRECTOR AND STAFF: $1084.00 This fee for our services and overhead will be added to the total cost of the funeral arrangements you select. (This fee is already included in our charge for direct cremation, immediate burials, and forwarding and receiving remains.) Personnel available 24 hours a day, 365 days a year, to respond to the initial call. Arrangement Conference Coordinating service with the cemetery, crematory, and/or other parties in the final Disposition of the deceased. Clerical assistance in the completion of various forms associated with the funeral. Securing and recording the death certificate and disposition permit as required Also covers overhead, including maintenance, equipment and inventory cost, insurance, administration, and governmental compliance PREPARATION: Except in certain special cases, embalming is not required by law. Embalming may be necessary, however, if you select certain funeral arrangements, such as viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as direct cremation or immediate burial. Our services include set-up of visitation area, placement of encased remains, display of floral arrangements, supervision of and attendance during visitation. $196.00 Use of facilities and Staff services in our Chapel. Our services include coordination and supervision of Service. $225.00 Staff Services for funeral at Other Facilities. Our Services include coordination and supervision of service $225.00 Use of Facilities and Staff for Memorial Services (Without the body present) at our Chapel. Our Services include coordinating the Memorial Service Arrangements, Supervision of the Memorial Service and staff to attend service, displaying of floral arrangements. $225.00 Staff Services for Memorial Service (Without the body present) at Other Facilities. Our Services include Coordinating of the Memorial Service Arrangements, supervision of memorial service and staff to attend Services . $225.00 Staff for Graveside Service. Our Services include accompaniment of remains to cemetery. Supervision of graveside service and placement of floral arrangements. Remaining for grave closure $255.00 TRANSPORTATION: Local transfer of remains from place of death to mortuary. Up to 100 mile radius. Additional distance at the rate of $1.75 per mile $100.00 Funeral Vehicle (e.g. Hearse) for funeral service. $1.75 per mile beyond local service area. $100.00 Family Vehicle, lead car for minister or used for casketbearers. (per vehicle) $100.00 Service Vehicle $ 75.00 Transfer to or from Airport $1.45 per mile various FORWARDING REMAINS TO ANOTHER FUNERAL HOME: This charge includes local removal of remains, minimum staff services, necessary authorizations, embalming, use of preparation room. $1102.00 RECEIVING REMAINS FROM ANOTHER FUNERAL HOME: This charge includes minimum services of staff, and local transportation. This charge does not include visitation rites or ceremonies. $ 782.00 DIRECT CREMATION: If you wish to arrange a direct cremation, you can use an unfinished wood box or an Alternative Container. Alternative Containers encase the body and can be made from materials like fiberbroad or composition material with or without an outside covering. The Alternative Container we use is a wood base with a cardboard cover. $4980-$1770.00 Our charges for direct cremation (without ceremony) include: Local removal of remains, transportation to a local crematory, Arrangement conference, securing coroner permit, minimum staff services and the cremation fee. 1. Direct Cremation with the Outer Container provided by purchaser (all containers must meet State and Crematory requirements). $1700.00 2. Direct cremation with a Alternative Container supplied by Funeral Home $1850.00 IMMEDIATE BURIAL: Our charges for immediate burial (without any rites or ceremonies at the Funeral Home, Graveside or Elsewhere) include: Local removal of remains from place of death, transportation to cemetery and minimum staff services. 1. Immediate burial with container provided by purchaser (all such containers must meet applicable State and Cemetery requirements) $1200.00 2. Immediate burial with minimum casket. $1916.00 3.
